Free T4 (FT4) Test / Thyroxine Test

Thyroxine (T4) is the primary hormone of the thyroid gland. It plays an important role in growth and metabolism. T4 exists in two forms in the body: freeT4 and T4 bound to proteins. Most of the T4 is bound to proteins.


There are two types of tests to measure T4: a total T4 test that measures both bound and free T4 and a free T4 test that measures the levels of free T4 in the blood.

Why is a free T4 test performed?

The test may be performed to assess thyroid function and diagnose thyroid diseases.

When is the test recommended?

The test may be advised when a person experiences the signs and symptoms of hypothyroidism or hyperthyroidism, especially when the TSH levels are abnormal. The test may also be useful in distinguishing thyroid disorders, diagnosing pituitary disorders and monitoring treatment.

How is the sample collected?

A blood sample is collected from the vein in the arm of the individual using a needle.

How to prepare for the test?

No special preparation is required for the test. However, remember to inform the doctor about all the medicines you are taking before taking the test as some medicines may affect test results. Those who are taking hormones for thyroid treatment should provide their blood samples before taking their dose for the day.
